- Maintain open communication with department directors and supervisors regarding program needs and developments.
- Provide insights to the Healthcare Home Director on regional issues and necessary actions for Continuous Quality Improvement.
- Assist clinical teams in mastering essential software for healthcare home data management.
- Oversee training on relevant health topics for staff members.
- Ensure compliance with metabolic syndrome screening goals and manage supply inventory.
- Lead and support a team of Nurse Care Managers, providing both administrative and clinical oversight.
- Facilitate onboarding of new personnel and disseminate critical policy information to staff.
