Summary
As an Advanced Practice Provider at Compassion Pathway Behavioral Health, you will play a vital role in delivering high-quality mental health care to our clients. This position is essential in supporting our mission to reduce hospital readmissions through client-centered rehabilitation programs.
Responsibilities
- Conduct comprehensive assessments of clients' mental health needs.
- Administer medications and monitor patient responses to treatment.
- Collaborate with interdisciplinary teams to develop individualized treatment plans.
- Provide outpatient care and follow-up support for clients in various settings.
- Educate clients and families about mental health conditions and treatment options.
- Ensure compliance with clinical standards and best practices in mental health care.
- Participate in utilization management activities to optimize patient care outcomes.
- Document all patient interactions accurately and timely.
Qualifications
- Valid certification as an Advanced Practice Provider or equivalent in the mental health field.
- Experience in outpatient mental health settings is preferred.
- Strong skills in medication administration and monitoring.
- Knowledge of critical care principles related to mental health is beneficial.
- Excellent communication skills for effective collaboration with clients and team members.
- Ability to work independently while managing multiple cases efficiently.
