Job Overview
We are looking for a reliable and detail-oriented Care Coordinator to join our team. This position involves a variety of tasks aimed at ensuring the quality of care for our clients. Key Responsibilities:
- Perform spot checks to ensure adherence to care protocols.
- Conduct documentation checks and audits to maintain accurate and compliant records.
- Oversee caregiver performance and conduct periodic pop-in visits for quality assurance.
- Monitor and manage inventory of care supplies to ensure availability.
- Regularly check in with clients to ensure their needs are being met.
- Flexible work hours to accommodate varying client needs and schedules.
- Ensure compliance with HIPAA regulations in all aspects of care coordination.
Experience & Qualifications:
- Preferred: LVN (Licensed Vocational Nurse) license, though not required.
- Minimum requirement: High school diploma or GED.
- CPR (Cardiopulmonary Resuscitation) and BLS (Basic Life Support) certification required
- Basic knowledge of medical terminology and human anatomy is preferred.
- Looking for candidates with a background in the medical field, those currently in nursing school, or individuals with experience in home care or caregiving.
- Strong attention to detail and ability to manage multiple tasks simultaneously.
- Ability to work independently and as part of a team in a dynamic environment.
- Must have reliable transportation to travel between client locations
