What are the responsibilities and job description for the Hospice Admission RN position at Compassionate Hospice Care?
The Hospice Admissions Nurse is a registered nurse who primarily will be responsible for performing evaluations and assessment for patients to determine if they meet hospice criteria. The Hospice Admissions Nurse uses their knowledge in pain and symptom management and knowledge of end-of-life processes in order to educate these patients, and family members.
Responsibilities:
- Evaluates and determines if patients meet hospice criteria
- Admits patients to hospice services
- Conducts initial meetings with patients and families to inform them about
hospice services and obtains consent for care
- Performs follow up meetings and phone calls from information visits
- Documents visits and phone calls with patients, families and referral sources
- May require additional nursing visits to support Case Managers based on census
- Mentors and trains new registered nurses
Job Types: Part-time, PRN, Per diem
Pay: $30.36 - $50.14 per hour
