Job Summary:
As an External Case Manager with Complete Care at Berkshire, you will play a key role in ensuring smooth transitions for patients from hospitals to our post-acute care facility. Responsibilities & Qualifications of an External Case Manager:
- Assess patients being discharged from hospitals to determine their care needs in the post-acute setting.
- Collaborate with hospital staff, patients, and families to develop comprehensive transition plans.
- Facilitate effective communication between hospital and facility staff, patients, and families.
- Provide patient and family education.
- Identify opportunities for increased market share from assigned hospital accounts.
- Maintain excellent organizational skills.
- Ensure timely communication and acceptance of hospital referrals.
Qualifications:
- Bachelor’s degree in healthcare administration, business, or a related field (preferred).
- 3-5 years of experience as an Admissions Director, Hospital Liaison in a skilled nursing/LTACH, or acute rehab hospital.
- Strong communication and interpersonal skills.
- Knowledge of healthcare regulations and standards.
