Your Role as an RN Case Manager
As a trusted advisor and advocate, you will ensure our clients receive top-tier, personalized care in the comfort of their homes. This role is 50% office-based and 50% field-based, visiting current clients and performing starts of care for new clients.
Key Responsibilities:
Conduct comprehensive client assessments and create customized care plans.
Assess and recommend additional care services to enhance client well-being and company revenue.
Supervise and support caregivers, providing training and performance evaluations (90-day and annual reviews).
Collaborate with families, caregivers, and healthcare providers to ensure seamless, high-quality care.
Educate clients and families on health management, disease prevention, and self-care strategies.
Maintain documentation in compliance with regulatory standards.
Participate in team meetings, case conferences, and professional development sessions.
What You Bring to the Team:
Current RN license in GA (required)
Minimum 5 years of clinical nursing experience (required)
2 years of Clinical Management experience in Home Care, Home Health, or a related setting (recommended)
Strong clinical assessment, critical thinking, and communication skills
Ability to independently manage cases while working within a supportive team environment
Reliable transportation for travel within the metro Atlanta area
BLS certification and proficiency in Electronic Health Records (EHR) systems
