Complete Care Management is seeking a dedicated and proactive External Care Manager to join our team in the Phillipsburg, NJ market. As an External Care Manager, you will play a critical role in ensuring seamless transitions of care and building strong relationships with healthcare providers, patients, and families.
Key Responsibilities:
- Collaborate with hospitals, physicians, and other healthcare providers to facilitate patient admissions to Complete Care facilities.
- Serve as a liaison between the facility and external partners, ensuring excellent communication and care coordination.
- Assess patient needs and provide guidance to families regarding services and care plans.
- Monitor and manage patient transitions to minimize gaps in care.
- Build and maintain relationships with referral sources to drive census growth and positive community engagement.
- Educate external partners on Complete Care’s services and capabilities.
- Track and report key metrics related to referrals and admissions.
Qualifications:
- Bachelor’s degree in healthcare, social work, or a related field preferred.
- Experience in care management, discharge planning, or a similar role in a healthcare setting.
- Strong interpersonal and communication skills.
- Ability to work independently and collaboratively in a fast-paced environment.
- Knowledge of skilled nursing and rehabilitation services is a plus.
