Key Responsibilities :
- Collaborate with hospitals, physicians, and other healthcare providers to facilitate patient admissions to Complete Care facilities.
- Serve as a liaison between the facility and external partners, ensuring excellent communication and care coordination.
- Assess patient needs and provide guidance to families regarding services and care plans.
- Monitor and manage patient transitions to minimize gaps in care.
- Build and maintain relationships with referral sources to drive census growth and positive community engagement.
- Educate external partners on Complete Care’s services and capabilities.
- Track and report key metrics related to referrals and admissions.
