Responsibilities:
- Conduct comprehensive assessments of patients' occupational performance, identifying areas of impairment and formulating realistic therapeutic goals.
- Develop individualized treatment plans based on assessment findings, focusing on activities of daily living (ADLs), adaptive equipment, and environmental modifications to improve patient functionality.
- Provide skilled occupational therapy services to patients in accordance with physician orders, addressing issues such as fine motor skills, cognitive function, and sensory integration.
- Collaborate with interdisciplinary team members, including nurses, physical therapists, and other healthcare professionals, to ensure coordinated and holistic patient care.
- Educate patients and their families on adaptive techniques, energy conservation, and strategies to enhance independence in daily activities.
- Instruct and supervise patients in prescribed therapeutic exercises and activities to improve motor coordination and overall functional abilities.
- Document patient progress, treatment interventions, and changes in the patient's condition accurately and timely in accordance with regulatory standards.
- Participate in case conferences and team meetings to discuss patient care plans and contribute to the overall care coordination.
- Stay abreast of current occupational therapy practices, emerging trends, and home health regulations to provide evidence-based and up-to-date care.
- Adhere to ethical and professional standards, ensuring patient confidentiality and fostering a supportive and caring environment.
Qualifications:
- Valid state license as an Occupational Therapist.
- Minimum of 1 years of experience in home health occupational therapy and/or in an acute care setting.
- Proficient clinical skills with a focus on patient-centered care.
- Excellent communication and interpersonal skills.
- Ability to work independently and collaboratively within a team.
- Understanding of Medicare home health regulations and documentation requirements.
- Current CPR certification.
- Reliable transportation for home visits.
