Position Overview: The primary duty is to assist individuals to maximize their goals developed on their PCP Plan. Supports the individuals served to build natural supports and relationships in the community. Ensures the health, safety and well-being of individual(s) served. This position is supervised by either the Associate Professional and/or Qualified Professional of the program.
Essential Duties and Responsibilities
Administrative:
- Prepares and submits required requests for reimbursement of work-related mileage, time sheets, or requests for use of Paid Time Off in an accurate and timely manner.
- Employee in this position must work their assigned schedule.
- Attends routine and called staff meetings.
Financial:
- Adheres to financial policies and procedures of the program and company.
Supervision:
- Not applicable.
Regulatory:
- Ensures and maintains a clean and safe work area.
- Adheres to all regulatory standards (county, state and federal, HIPAA, MH/DD/SA, DHSR, DHHS, CARF) and ComServ, Inc. policies and procedures.
- Reports any violations of regulations to direct supervisor.
Documentation:
- Documents administration of medications on the Medication Administration Record (MAR) when responsible for administering medications.
- Documents services rendered accurately in the required format within established timeframes as specified by the services rules and regulations that are being provided.
- Documents all other required information. (i.e.: individuals timesheets and piece rates, medication counts, etc.)
- Accurately documents incidents reports in the required time frame.
Clinical:
- Trains the individuals in knowledge of their human rights and how to advocate for those rights.
- Advocates for the individuals in the protection of their human rights.
- Reports concerns and problems to the supervisor in a timely manner and actively participates in problem resolution.
- Develops an extensive knowledge of the Person Centered Plan for each individual in order to implement the goals and ensures that he/she receives services as specified in the PCP. Reports any problems or requests for new programs/goals to the supervisor in a timely manner.
- Coordinates information pertaining to the individual(s)/program(s) served with supervisor.
- Coordinates all communications regarding the individuals care via the supervisor.
- Implements active treatment for all individuals.
- Complies with all aspects of the clinical supervisory plan.
- Has an understanding of medications that the individual takes and brings any concerns to the supervisor.
- Participates in development of goals/PCP Plan.
Direct Support:
- Assists individuals with the opportunity to interact and become a part of the community. Assist in making other personal choices appropriate for their age.
- Provides adaptive skill training for individuals as identified in the Person Centered Plan. This may include but not be limited to:
- Providing Personal Care: bathing, toileting, tooth brushing, feeding (based on level of functioning)
- Providing Daily Living: laundry, meal preparation, cleaning, safety, shopping, choice making, exercising
- Providing Social Skills: hand shaking, interactions, communication
- Providing Leisure/Recreational: exercising, community integration
- Providing Budgeting/Financial: identify money
- Provides meaningful activities based on individual differences and level of functioning
- Follows behavioral interventions (e.g. physical restraint, de-escalation, re-direction) as specified in the Person Centered Plan and behavior support/crisis plans.
- Provides supportive counseling for individuals as identified in the Person Centered Plan. This may include, but not limited to anger management, problem solving, psychosocial skills (self-esteem, identity and sense of self), and conflict resolution.
- Acts as a role model for individuals.
- Ensures the health and safety of individuals at all times.
- Completes meal preparations or assist with lunch preparations depending on the setting of the job.
- Provides meaningful activities based on individual differences and level of functioning.
- Provides supervision of individuals at all times.
- Manages crisis situations by implementing the individual crisis plan/behavior support plan.
- Provides individual served with dignity and respect as an adult.
Other:
- Provides transportation for individuals as needed for approved activities.
- Attends all organizational and outside training as required by the position.
- Attends any outside training and education as approved or assigned by supervisor.
- Employee in this position must be flexible with schedule and makes changes as requested or required by position responsibilities, such as working different shifts, holidays, earlier or later hours.
- Ensures and supports cultural competency in conducting responsibilities that is reflective of the community and individuals served in regard to culture, age, gender, sexual orientation, spiritual beliefs, socioeconomic status, and language.
