Support, Inc. is primarily a child/adolescent mental health and substance use provider serving Gaston, Cleveland, Lincoln, and other surrounding counties for over 20 years. Support, Inc. provides the full array of child/adolescent mental health and substance abuse services, with the exception of level 3 and PRTF services.
Support Incorporated is seeking a supervisor for our Gastonia Day Treatment. This applicant should have leadership and supervisory experience and be considered a Qualified Professional (related bachelor's degree with two years of experience with the child and adolescent mental health population or a bachelor's degree that is not related and four years of experience with the population served).
Description of Responsibilities & Duties:
Responsible for leading an established day treatment program with approximately 12 full-time staff.
Serve as liaison to the local school system and be the face of our day treatment in Lincoln County.
Work closely with clinicians, teachers, and mental health professionals to implement a therapeutic milieu from 8am to 3pm, Monday through Friday.
Ensure educational and treatment needs of the clients are met. This includes being responsible for the implementation of best practice treatments/models.
Completes orientation to services for new clients and assists with required documentation and submitting for service authorizations.
Provide case management services including, but not limited to, assessing the need for comprehensive services, linking the child/family to needed services/supports, monitoring the provision of services/supports, convening a Child/Family Team meeting to coordinate the provision of multiple services and ensure appropriate modification of the Person-Centered Plan.
Assist with updating, implementing, and reviewing Person Centered Plans (PCP) goals with the consumer, family, and treatment team members and ensure coordination of this plan with other plans of the consumer life (e.g., IEP, 504 Plan, Probationary).
Develop transition plans. Follow-up is required for all planned and unplanned discharges. The service record must reflect outcomes sustained and progress toward implementing the transition plan.
When appropriate, coordinate therapeutic interventions away from the program in places related to achieving service goals, including, but not limited to, normalizing community activities and group or play-based therapies in the consumer's natural setting.
Assure that all appropriate clinical documentation, including but not limited to person-centered plans, assessments, consents, screenings, applications, and authorizations, are maintained in accordance with Federal and State requirements.
Supervise employees, including but not limited to hiring, evaluating, credentialing, disciplining, training in accordance with day treatment supervises, and other personnel actions throughout their employment with the Agency.
