What are the responsibilities and job description for the Case Manager - Scatter Site Programs all boroughs position at Comunilife?
Comunilife is looking for a Human Services professionals who are committed to provide case management services to our clients in a dignified manner. Our Office of Mental Health (OMH) and Human Resource Administration (HRA) scatter site supported housing programs provide housing and case management services for seriously mentally ill adults. Participants are referred from state psychiatric institutions, community residences, hospitals, and from local shelters and agencies. The program secures permanent, supported housing for participants and provides them with case management and related services. Case managers are responsible for conducting home visits to client's apartments. The positions are eligible to participate in Comunilife's generous benefits and paid time off package within 3 months of hire. The salary range for these positions are between $40,000 and $45,000 a year.
Essential Functions
· Completing comprehensive assessments, service plans, progress notes
· Monitoring and reassess clients’ needs for referrals and services
· Provide psychoeducation on relevant topics like substance use disorders, entitlements, etc.
· Provides individual and group workshops
· Work collaboratively in an inter-disciplinary team to ensure comprehensive, client-centered services are delivered and documented
REQUIRED JOB QUALIFICATIONS:
Education and Experience Requirement(s):
- Bachelor’s Degree in human services or related field
- Two (2) years’ work experience
- Experience with HIV/AIDS afflicted individuals, chronic mental illness, substance abuse individuals and their families a plus
- Bilingual skills in English/Spanish a plus
Knowledge, Skills, and Abilities Required:
- Understanding of and commitment to organization’s mission, vision, and values
- Demonstrated knowledge of compliance, evidence–based practices, and quality improvement tasks and processes to ensure that programs are operating consistently with regulations of funders and regulatory bodies
- Excellent oral and written communication skills; proficiency with MS Office and Google product suites
- Ability to perform and manage in a team approach
- Ability to handle complex challenges and balance multiple demands on time and attention
- Ability and willingness to perform all duties as workload necessitates
- Community minded, patient, creative, flexible, and compassionate; culturally sensitive to the populations served by Comunilife
- Demonstrates flexible and efficient time management and ability to prioritize
- Knowledge of systems such as AWARDS, CAIRS, CARES, etc.
