Home Visit Specialist - Part-Time
Job Purpose
The Home Visit Specialist conducts home visits to identify barriers, disseminate information, and strengthen relationships between families, schools, and students. The home visit specialist work is guided by the Concentric Educational Solutions Home Visit Framework. Our framework takes the punity out of responding to student needs. The home visit specialist’s responsibility is to conduct in-person home visits and act as a liaison between the family and school-based personnel.
Duties and responsibilities
The primary job duties and responsibilities of the home visit specialist are:
- Document home visit outcomes in the Home Visit and Student Engagement App
- Use communication and relationship-building skills to connect with families
- Share information about school and district-based resources
- Provide information about important dates and deadlines, any missing documentation (i.e. immunization records), and/or deliver materials (welcome packets, devices, food, etc.).
- Attend pre and post-home visit meetings with schools and districts as needed
Qualifications:
- Strong written and verbal communication skills
- Valid driver’s license
- Clean driving record
- Valid car insurance
- Must have a cell phone and/or tablet with internet access
- Must be able to work evenings (no later than 8:00 PM) and weekends
- Able to work a minimum of 25 hrs. per week
- Reliable and punctual person
Preferred Qualifications include:
- At least an Associates Degree
- Prior experience with documentation and note-taking (Behavioral Specialist, Social Worker, Probation Officer)
- Prior experience with conducting home or site visits (Social Worker, Probation Officer)
- Comfortable and skilled with technology (Web-based browsers, Google Meet, Google Docs, Google Sheets, Zoom)
