Home Visit Specialist – Part-Time

Concentric Educational Solutions (CES) is seeking dedicated and compassionate Home Visit Specialists to join our team. In this role, you will conduct in-person home visits to build relationships, identify barriers to student success, and serve as a bridge between families, schools, and districts.

Your Role & Responsibilities
As a Home Visit Specialist, you will :
Conduct in-person home visits to connect with families and provide crucial support.
Use communication and relationship-building skills to foster trust and engagement.
Share important school and district resources with families.
Provide reminders about key deadlines, missing documents (e.g., immunization records), and deliver materials such as welcome packets, devices, and food when needed.
Document all home visit outcomes in the Home Visit and Student Engagement App.
Attend pre- and post-visit meetings with school and district personnel as needed.
