What are the responsibilities and job description for the SCL Hourly Counselor position at Concerned Inc?
The Hours for this position vary based on the needs of our members and their schedules. The busiest times however are currently 3 pm to 9 pm. Every other weekend and holiday are required.
Employees are paid on a weekly basis
We currently offer a $1,500 sign on bonus.
Purpose of the job:
The Supported Community Living Hourly Counselorwill be responsible for implementing daily living skills and other activities as they relate to the member's Individual Treatment Plan (ITP).
Essential functions:

Duties and responsibilities:
1. Teaches, trains, and assists members in developing the skills necessary to be as independent as possible when performing activities of daily living. These will include:
a. Training, if needed, members to take care of their own health and medical needs. Communicates any changes of these needs to Hourly Team Leader to discuss alternative methods of training and communication.
b. Assisting members to develop or maintain skills to independently access community services and to participate in their community.
c. Assisting members in developing skills to advocate for their rights and to make informed choices.
d. Assisting members in preparing an individualized diet plan to meet their nutritional needs.
e. Training members to be responsible for their safety in an emergency by conducting health and safety activities (safety drills/training, etc.).
f. Working with members to personally manage their personal business and property.
g. Assisting members with monthly budgeting
i. Monitoring weekly personal allowances
1. Filling out money requests
2. Ensures that receipts get turned into the SCL Hourly Team Leader
ii. Giving guidance in budgeting personal needs and desires.
1. Ensuring money requests accompany all needs/desires for money.
iii. Communicates member wants and needs to the Hourly Team Leader.
h. Communicate with SCL Coordinator regarding assisting members to apply for government assistance programs including HUD, Food Stamps, Heat assistance, Rent Reimbursements, etc.
i. Providing transportation to assist the members in obtaining services for carrying out activities of daily living such as grocery shopping, doctor appointments etc. and for community integration.
j. Providing guidance for daily living skills including cleaning, laundry, and meal preparation.
2. Assesses member’s skills and provides input for the Person-Centered Plan.
a. Communicate skills and input to Hourly Team Leader and SCL Coordinator regarding goal suggestions.
3. Completes member’s program goals and data collection related to the goals.
4. Supervises members in the care and use of clothing and personal property and assists with clothing purchases, as necessary.
5. Ensures that all persons served by the program that require 24-hour care are not left unattended.
6. Follows the assigned work schedule keeping hours scheduled to work. Contacts the SCL Hourly Department if unable to attend work in a timely manner.
7. Attends staff meetings and scheduled in-service training sessions.
8. Documents daily in PCP’s, diet menus, activities, and any other documentation necessary.
9. Ensures that SCL Hourly member medications are filled, picked up, set up, and medication goals are being completed. Administers medication correctly and maintains accurate documentation in the med logs.
10. Suggest ideas in programming to the SCL Coordinator.
11. Attends seminars, meetings, and training as deemed necessary.
12. Completes necessary agency reports in a timely manner.
13. Performs other duties as assigned.
Supervisory responsibilities:
- The SCL Hourly Counselor supervises members when working with members on an hourly basis.
Knowledge, skills, and abilities:
Requirements:
1. High School Diploma or equivalent.
2. Demonstrates computer application skills.
3. Shall secure a medication manager certificate upon employment.
4. Must be able to read, speak, and write in English.
5. Holds a valid State of Iowa Driver’s License and is able to travel (must be willing to drive in urban areas to meet member needs, attend training, etc.).
6. Provides proof of valid vehicle insurance.
7. Submits to a state criminal record check. Employment is contingent upon a clear record.
8. Submits to dependent adult and child abuse record checks. Employment is contingent upon a clear record for both checks.
9. Submits to a state driver’s license check. Employment is contingent upon a clear record.
10. Must secure Class D Chauffer’s license upon hire.
Fiscal responsibilities:
1. Maintains an accurate time sheet and submits to SCL Department in a timely manner.
2. Assists members in budgeting their money on hand.
Physical Demands:
1. Lifts 40 lbs.
2. Obtains a physical examination from a licensed physician upon offer of employment.
3. Manual dexterity to include, but not limited to, the following: stooping, bending, lifting, and carrying.
Evaluation:
The Supported Community Living Hourly Counselor will be hired, supervised, evaluated, and terminated by the SCL Coordinator. This employee shall be evaluated at 90 days and at one-year intervals thereafter.
