Employees are paid on a weekly basis.
Currently offering a $1,500 sign on bonus.
The hours for this position are M-F from 3 pm to 11 pm and 8 am to 8 pm or 8 pm to 8 am every other weekend and holiday.
Purpose of the job:
The Supported Community Living Site Counselorwill be responsible for implementing daily living skills and other activities as they relate to the member's Individual Treatment Plan (ITP).
Essential functions:

Duties and responsibilities:
1. Teaches, trains, and assists individuals in developing the skills necessary to be as independent as possible when performing activities of daily living. These will include:
a. Training, if needed, individuals to take care of his/her own health and medical needs. Communicates changes to SCL Site Team Leader.
b. Assisting members to develop or maintain skills to independently access community services and to participate in their community.
c. Assisting individuals in developing skills to advocate for their rights and to make informed choices.
d. Assisting individuals in preparing an individualized diet plan to meet their nutritional needs.
e. Training individuals to be responsible for themselves in an emergency by conducting health and safety activities (safety drills/training, etc.).
f. Working with individuals to personally manage their personal business and property.
g. Assisting members with monthly budgeting by monitoring weekly personal allowances to ensure that they stay within their budgets and giving guidance in budgeting personal needs and desires. Communicates member wants and needs to the SCL Coordinator.
h. Providing transportation to assist the individual in obtaining services for carrying out activities of daily living such as grocery shopping, doctor appointments, community integration, etc.
i. Providing guidance for daily living skills including cleaning, laundry, and meal preparation.
2. Orients new members to new living situations.
3. Completes safety, health, and nutrition assessments.
4. Assesses member’s skills and provides input to the PCP.
5. Documents daily per PCP’s, diet menus, activities, and any other documentation necessary.
6. Completes individual’s program goals and data collection related to the goals.
7. Supervises individuals in the care and use of clothing and personal property and assists with clothing purchases as necessary.
8. Ensures that all persons served by the program that require 24-hour care are not left unattended.
9. Follows the assigned work schedule keeping hours scheduled to work. Contacts the SCL Site Department if unable to attend work in a timely manner.
10. Attends staff meetings and scheduled in-service training sessions.
11. Ensures that the Concerned, Inc. residences meet minimum cleanliness, safety, and health standards as set by the agency.
12. Keeps informed of all programming and administrative information, changes, and additions by checking the daily log and/or other messages at all sites.
13. Administers medication correctly and maintains accurate medication documentation.
14. Sets up medications with the members when needed.
15. Suggest ideas in programming to the SCL Coordinator.
16. Attends seminars, meetings, and training as deemed necessary.
17. Completes necessary agency reports in a timely manner.
18. Performs other duties as assigned.
Supervisory responsibilities:
- The SCL Site Counselor supervises members when on duty at residential sites.
Knowledge, skills, and abilities:
Requirements:
1. High School Diploma or equivalent.
2. Demonstrates computer application skills.
3. Shall secure a medication manager certificate upon employment.
4. Must be able to read, speak, and write in English.
5. Holds a valid State of Iowa Driver’s License and is able to travel (must be willing to drive in urban areas to meet member needs, attend training, etc.).
6. Provides proof of valid vehicle insurance.
7. Submits to a state criminal record check. Employment is contingent upon a clear record.
8. Submits to dependent adult and child abuse record checks. Employment is contingent upon a clear record for both checks.
9. Submits to a state driver’s license check. Employment is contingent upon a clear record.
10. Must secure Class D Chauffer’s license upon hire.
Fiscal responsibilities:
1. Maintains an accurate time sheet and submits to SCL Department in a timely manner.
2. Assists members in budgeting their money on hand.
Physical Demands:
1. Lifts 40 lbs.
2. Obtains a physical examination from a licensed physician upon offer of employment.
3. Manual dexterity to include, but not limited to, the following: stooping, bending, lifting, and carrying.
Evaluation:
The Supported Community Living Site Counselor will be hired, supervised, evaluated, and terminated by the SCL Coordinator. This employee shall be evaluated at 90 days and at one-year intervals thereafter.
