General Description:
The Permanency Coordinator provides oversees adoption services under contract from the Statewide Adoption Network. This position works in collaboration with The Philadelphia Department of Human Services and Community Umbrella Agencies. The Permanency Coordinator’s role is to: (1) supervise services adoptive families and children throughout the adoption process through direct supervision of permanency consultants; (2) maintain case records with timely and accurate documentation meeting state/county standards and best practices and (3) ensure accountable adoption placements for children. The Permanency Coordinator has access to confidential client information and is expected to handle such information with integrity and professionalism.
Qualifications
Duties and Responsibilities:
- Supervise and monitor the provision of resource services to pre and post adoptive children and families and natural birth parents through reunification.
- Ensure quality supervision of all permanency staff, including consultants.
- Responsible for qualifying families and children for adoption placement.
- Establish a working team relationship with placement families to ensure child’s well-being and promote positive growth and development.
- Maintain case records with timely and accurate documentation meeting state/county standards and best practices.
- Ensure billing goals are met through the submission of monthly invoices for SWAN services.
- Oversee the completion of child/family profiles, child preparation, adoption finalization services.
- Appear at court hearings as required.
- Responsible for providing accountable adoption service completion within each fiscal/contract year.
- Participate in weekly program meetings and monthly staff meetings.
- Participate in case reviews, as scheduled for individual cases, and present case issues.
- Attend trainings as required.
- Perform other duties as assigned by the Assistant Director or Director
- Maintain on-going mutually supportive, professional collaboration with the Philadelphia Department of Human Services, other state/county agencies, auxiliary services and community resources.
Internal & External Relationships

- Engage with funders and partners and represent Concilio as needed or as assigned.
- Ensure high quality representation in conferences and other events.
- Perform other duties as assigned.
Qualifications/Requirements
- bachelor’s degree with 2 years in related field required, master’s degree in social work or related field preferred
- A minimum of two years work and supervisory experience in direct services related to adults, families and children, preferably child welfare.
- FBI, Pennsylvania Childline and Criminal Background Clearances.
- Bilingual English/Spanish preferred
- Must have the ability to be flexible and work extended hours and some weekends.
- Excellent oral and written communication skills.
- Excellent analytical ability.
- Computer and technological proficiency required.
- Ability to work under pressure and handle emergency situations.

Salary : $50,000 - $55,000
