Concordia at Weatherwood is seeking a dedicated and quality focused Licensed Practical Nurse (LPN) to be their next Resident Care Coordinator.
The Resident Care Coordinator is responsible to assure the provision of high quality care, compliance and customer service to their residents and a leader to the staff.
This position offers competitive rates and the salary will commensurate with experience.

Qualifications of a Resident Care Coordinator:

• Licensed Practical Nurse (LPN) will valid current Pennsylvania license
• Excellent verbal and written communication skills
• Supervisory experience is a plus
• Knowledge of or ability to learn Department of Human Services Personal Care Home guidelines

Resident Care Coordinator's Responsibilities include but are not limited to:

• Ensures the highest quality of care to the residents is provided by staying up-to-date with daily reports, doctor visits, assessments and is knowledgeable of resident care outcome trends
• Assists the administrator in implementing systems to assure compliance with all applicable local, state and federal regulations and accreditation standards
• Assists the administrator with the annual budge process
• Assists the administrator in all aspects of human resource management
• Establishes and builds excellent relationships and communications with residents, families, employees and community members
• Commits to high ethical standards and complies with the code of conduct
