Concordia Visiting Nurses is a hiring a Full-time Triage/Weekend Workflow Supervisor. The schedule for this position varies but consists of a mix of weekdays and weekends, daylight and overnight hours.
POSITION SUMMARY
The Triage/Weekend Workflow Supervisor is a licensed health care professional that oversees the day-to-day operations of the Triage/Weekend Workflow Teams. These teams receive and assess telephone calls from patients, families, and referral sources to advise and coordinate the level of decision making required from the communication. It is the responsibility of the triage/weekend workflow nurse to determine the medical assessment and/or education required either by telephone conversation, nursing visit or communication to other service lines or departments the next day. The triage/weekend workflow nurse collaborates with all Interdisciplinary Group members and facilitates communication between the home health teams ospiceand the patient’s attending physician consistent with the patient’s Plan of Care.
QUALIFICATIONS
Current Registered Nurse (RN) license in the state of Pennsylvania
Knowledge of OASIS guidance required
Minimum of [2] years of current experience as a professional nurse in a medical/surgical environment and [1] year home health experience preferred
Possesses strong assessment, problem solving and decision-making skills
Expertise in pain management, symptom control, crisis intervention, and family support
Solid computer skills, including Word, Excel and electronic medical record (EMR) programs
Excellent organizational skills with attention to detail
Outstanding communication skills (both written and verbal); must communicate in a warm, professional manner
Reliable independent means of transportation with a valid Pennsylvania driver’s license and at least the minimum state required vehicle insurance
Knowledge base of agency policies, federal and state regulations, and third-party payers
RESPONSIBILITIES
Supervises day to day operations of the triage/weekend workflow team.
Researches concerns/issues in a timely manner
Performs one on one training for team members upon hire and at regular intervals to ensure that work performance and customer service standards are being met
Conducts annual performance evaluations of all direct reports
Responsible for payroll and approval of time sheets and assists with leave of absence tracking
Creates and maintains monthly employee schedules and daily task assignments
Responsible for progressive disciplinary and counseling for employees, as needed
Triages and helps patients, caregivers/families, interdisciplinary team members and health care providers as it relates to patient problems, concerns, and/or questions
Manages patient’s symptoms pursuant to physician orders; obtains changes in orders as needed and updates the patient’s Plan of Care and medication list accordingly
Assesses the patient/family’s need for a home visit; contacts the appropriate clinician to schedule the visit and maintains communication with the patient/family of the expected visit time
Guides, supports, and provides education to home health patients and families via telephone or virtual communication
Orders durable medical equipment and supplies for the patient as needed
Documents all communication services provided in the patient’s electronic medical record ensuring that all care provided is consistent with the patient’s Plan of Care
Attends and participates in staff meetings and other related activities when required
Completes required in-services; ensures all required continuing education credits are obtained for licensure renewal
Demonstrates an understanding of, home health policies and procedures, and participates in projects when requested
Monitors workflow for all entities and ensures completion when needed.
Holds meetings with the Triage/Weekend Workflow teams quarterly and as needed.
Obtains initial insurance authorization for home health care services, when required.
Reviews and approves the referral for accuracy and completeness in the EMR. Reviews clinical documentation for compliance and completeness in a timely manner.
Reviews OASIS and plan of care for accuracy and completeness.
Review and process clinical workflow tasks related to reviewing the first two tasks in the OASIS for timeliness and delivery of quality care.
Possesses knowledge about payer processes and requirements to bridge the gap between provider and payer and to effectively meet the needs of both, ultimately for the patient’s benefit.
