The Speech Language Pathologist SLP is responsible for evaluating, planning and implementing a speech/language treatment program for patients in their home or current residences. The Speech/Language Pathologist works as a member of an interdisciplinary rehabilitation team.
QUALIFICATIONS for the Speech Language Pathologist SLP position:
Master’s Degree in Speech-Language Pathology [may also be co-certified in Audiology]. Certified in Speech-Language Pathology by the American Speech-Language-Hearing Association.
Minimum of one-year clinical experience as a Speech/Language Pathologist. Two years’ experience recommended.
Licensed as a Speech-Language Pathologist in the commonwealth of Pennsylvania
Reliable, independent means of transportation to make home visits. Drivers must have a valid driver’s license and drive a car with insurance limits as established in personnel policies
DUTIES AND RESPONSIBILITES for the Speech Language Pathologist SLP:
Evaluates patient’s speech and language abilities including cognitive, Dysphagia and auditory abilities, deficits and assets.
Plans and implements rehabilitative services for speech and language disorders, including cognitive, Dysphagia and auditory disorders
Develops the plan of care cooperatively with the physician, patient/family and other team members with revisions based on the patient’s condition
Instructs patients, families and other health care team members in the total speech therapy program including cognitive awareness and effective communication approaches.
Assigns and instructs a home health aide in accordance with a physician’s plan of care to assist with an established home program.
Supervises the assigned home health aide every 14 days when the speech therapist is the coordinating health professional. Plans with the patient, family and other team members for the patient’s discharge from service
