This is a hybrid position combining the roles of Activity Director and Social Service Director. You will be dividing your time for each role through the week as you see fit. Our current resident census is averaging around 40 residents. This allows for plenty of time to focus on both Social Services and Activities.

Qualifications:

  • Certification as an Activity Director from a State approved program or able to obtain.
  • CNA experience preferred but not required.
  • Degree in Social Work or related field OR A minimum of one (1) year of social service experience preferred
  • Long Term Care experience preferred.

  • Possess the necessary education and skills to interview, assess and develop individualized plans to meet residents’ needs
  • Must have the ability to listen to others and be creative in meeting those needs.
  • Maintain a positive attitude.
  • Display a general knowledge of Medicare, Medicaid and other insurance payers.
  • Communicate with residents, families, and staff in a professional and tactful manner.
  • Be a role model of professionalism and teamwork.
  • Must display a willingness to continue to study, learn and implement continuously changing state and federal regulations.
  • Completion of secondary education and must be able to read and write in a legible and understandable manner.
  • Experience with the geriatric patient and the long-term care setting.
  • Must maintain regular attendance. Must be flexible in work hours to offer adequate representation at events.
  • Must meet all local health regulations, and pass post-employment physical exam if required.
  • Must be capable of performing the essential job functions of this job, with or without reasonable accommodation.

Physical Requirements of Job:

  • Walking, reaching, bending, lifting, grasping, pushing and pulling, fine hand coordination, ability to hear and respond to pages, ability to understand and follow written and oral instructions and directions; ability to communicate with residents, families, personnel and consultants; and the ability to remain calm under stress.

Responsibilities:

Activities

  • Plan, schedule, and implement a program of individual and group activities based on residents' individual needs. Including but not limited to creating a monthly activity calendar and posting calendars in resident preferred areas timely.
  • Assess and complete the sections of MDS assigned completely, timely and accurately.
  • Use the resident assessment protocols (RAP's) to determine whether to proceed to care planning for concerns related to a resident's activity program needs.
  • Gather data and document required assessments.
  • Identify and document in the residents' care plan, the activity interests, needs, and concerns of the resident.
  • Document and maintain an activity participation records for each resident.
  • Participate as an active member of the interdisciplinary team and attend resident care conferences.
  • Provide in-service education programs and attend all in-service sessions relating to Activity Program.
  • Recruit, schedule and supervise volunteers.
  • Organize and schedule community events related to residents' interests.
  • Plan, schedule and implement room visits and in-room activities for residents unable to leave their rooms. Document the in-room visits.
  • Plan, schedule and implement activities for residents who prefer to direct their own activity program and make accessible supplies for those activities.
  • Plan, schedule and implement indoor and outdoor activity programs.
  • Assess needs and maintain an adequate supply of materials to implement activity programs.
  • Observe infection control procedures related the Activity Program, including identification related to residents not appropriate to attend out-of-the-room activity programs for health reasons.
  • Be responsible for the safety of residents under his/her supervision.
  • Follow all facility policies and procedures.
  • Follow Residents Rights policies at all times.

SOCIAL SERVICES

  • Implement social service policies and procedures to meet residents’ needs in compliance with federal, state and local regulations.
  • Develop, coordinate and participate in resident care plans. To ensure that information and care in the care plans is designed to promote social interaction, reality orientation and intellectual stimulation.
  • Document the social service component of the Care Plan in a timely manner.
  • Document psycho-social needs, interaction and follow-up action with resident and/or family according to facility policies and procedures.
  • Assess, plan and document residents’ discharge needs in accordance with facility discharge planning policies and regulatory requirements.
  • Develop one-to-one professional relationships with residents and families as needed for counseling. Counsel residents and families in dealing with feelings about death or dying and other emotional, mental, environmental or physical limitations.
  • Assess and complete the sections of the Minimum Data Set (MDS) assigned completely, timely and accurately.
  • Plan, schedule and implement room visits for residents unable to leave their rooms.
  • Act as a liaison with social, health and community agencies. Refer residents to social, health and community agencies and complete accurate documentation in residents’ records concerning the results of such referrals.
  • Coordinate family and community resources as needed to solve financial needs and to promote emotional security
  • Organize community events related to resident’s needs and interests. Coordinate and cooperate with the activity department to plan and conduct events to meet socialization needs of the residents.
  • To lead facility tours as needed.
  • Organize and participate in the abuse and neglect education.
  • To perform other duties as requested by management.
