The Medical Benefit Review Services Associate II offers an exciting opportunity to positively impact your career and community!
Responsibilities:
Conducting In-Home Assessments for LT-PCS applicants and participants.
Providing HCBS program choice counseling to requestors.
Providing an overview of the Long-Term Care Program to applicants (Rights and Responsibilities).
Developing a needs-based and Person-Centered Plan of Care.
Completing client monitoring surveys as part of quality management.
Requirements:
Bachelors in health/ human services, Social Work, Nursing or related.
2 years of health related social and/or human services experience.
Min 1-year exp working with older adults or persons with disabilities.
Must live in Louisiana.
Have a valid driver’s license.

For this position, actual salaries will vary and may be above or below the range based on various factors including but not limited to location, experience, and performance. In addition to base pay, this position, based on business need, may be eligible for a bonus or incentive. In addition, Conduent provides a variety of benefits to employees including health insurance coverage, voluntary dental and vision programs, life and disability insurance, a retirement savings plan, paid holidays, and paid time off (PTO) or vacation or sick time. The estimated salary range for this role is $19.47 per hour.
