Responsibilities :
Conducting In-Home Assessments for LT-PCS applicants and participants.
Providing HCBS program choice counseling to requestors.
Providing an overview of the Long-Term Care Program to applicants (Rights and Responsibilities).
Developing a needs-based and Person-Centered Plan of Care.
Completing client monitoring surveys as part of quality management.
Requirements :
Bachelors in health / human services, Social Work, Nursing or related.
2 years of health related social and / or human services experience.
Min 1-year exp working with older adults or persons with disabilities.
Must live in Louisiana.
Have a valid drivers license.

In addition to base pay, this position, based on business need, may be eligible for a bonus or incentive. The estimated salary range for this role is $19.47 per hour.
