Description:
High School Diploma or GED, which can be waived for Elders age 55 and older per tribal policies. Experience in customer services and the ability to talk with both Elders and program staff.
Description:
- Establish a regular visitation schedule for Elders within respective Area Office locations
- Visit Elders in their homes, over the telephone or in congregate settings..
- Keep a log of visits
- If any issues are identified, notify the appropriate staff member
- Provide a monthly report of activities or needs identified during visits with Elders
- Provide a formal report on a quarterly basis to the Social & Human Services Director to be included in the SHS Quarterly and annual reports.
