We are seeking an experienced and dynamic Clinical Manager to collaborate with and support the the clinical operations of a home health care agency. The Clinical Manager will play a key role in ensuring high-quality care, efficient operations, and regulatory compliance.
As a Clinical Manager, you will be responsible for leading a team of skilled clinicians, managing patient care programs, and working closely with interdisciplinary teams to ensure that all patients receive the best possible care at home.
What You'll Do:
- Receives case referrals and assigns appropriate clinician to case.
- Review patient's clinical diagnosis, medications, procedures, and clinical course.
- Assists in clinicians in establishing immediate and long term therapeutic goals, in setting priorities, and in developing plan of care.
- Conducts quarterly record reviews and communicates findings and recommendations to appropriate organization personnel.
- Assists in the planning, implementation, and evaluation of in-service and continuing education programs.
- Attends meetings, CEUs on a regular basis
What We're Looking For:
- Registered Nurse with current licensure to practice professional nursing in the state.
- Graduate of National League for Nursing accredited school of nursing. BSN degree is preferred.
- Two years management or supervisory experience in health care setting, preferably home care.
- Complies with accepted professional standards and practices.
- Complies and maintains current CPR certification.
- Demontrates excellent observation, verbal and written communication.
- Computer skills; must be able to work with EMR system.
