Position Details:
- Shift/s: Morning/Evening/Night
- Working days: Mon-Fri, and rotational weekends
- Guaranteed Hours: 32hr/week (4 days/week)
Compensation:
- Pay: $37.46/hr - $48.13/hr (commensurate experience/skills)
Responsibilities
- Licensed Practical Nurse Collect and document patient data.
- Monitor and report changes in patient conditions to the RN.
- Contribute to setting patient goals and developing care plans.
- Implement and evaluate interventions to improve outcomes.
- Administer medications and treatments as per physician orders.
- Accurately document care provided and communicate critical information.
- Oversee care provided by unlicensed staff and ensure compliance with policies.
- Coordinate with the interdisciplinary team for effective discharge planning.
- Participate in team meetings to enhance patient care.
- Adhere to all regulatory requirements and ethical standards, including HIPAA.
- Participate in quality assurance and improvement initiatives.
Requirement
- Graduate of an approved school for practical or vocational nursing is required.
- Active LPN license in NM.
- BLS/CPR certifications.
- Experience is preferred in rehabilitative or geriatric nursing.
