The Connecticut Harm Reduction Alliance (CTHRA) is a leading 501(c)(3) nonprofit
organization agency focusing on harm reduction services, helping support our large and growing
community across Connecticut, with locations in Hartford and New Haven. Our pillars focus on
educating, assisting, and supporting. Harm reduction is a set of practical strategies and ideas
aimed at reducing negative consequences associated with drug use. Harm reduction is not an
abstinence based program – we meet folks where they’re at. Our programs provide services to
people who use substances, engage in sex work, experiencing homlessness or unstable
housing, and/or individuals with limited access to healthcare.
CTHRA is seeking a Homeless Outreach Case Manager to support our Homeless Outreach
programs in the Greater Hartford region. The Homeless Outreach Case Manager will provide
outreach and engagement to individuals experiencing unsheltered homelessness, assessing
individual needs related to housing in order to assist households in finding appropriate, safe,
affordable housing opportunities, assisting households with meeting basic needs, client
advocacy, and ensuring individuals are successfully linked to resources as quickly as possible.
The Homeless Outreach Case Manager will be part of the team that canvasses during non-
traditional business hours.
General responsibilities, include but are not limited to:
- Work with team members to provide on-demand street-based response and
engagement with individuals experiencing substance use, mental health, and/or
unsheltered homelessness.
- Distribute and recover harm reduction supplies, and provide education and support on
safer drug use, safer sex, HIV, HCV, and STIs.
- During participant engagements, use brief assessment, active listening, and crisis
response skills to help meet immediate, basic needs, including need for shelter, food,
clothing, etc.
- Engage with individuals experiencing unsheltered homelessness to obtain documents in
preparation for housing services. Complete Homeless Verification forms and coordinate the
completion of Disabling Condition Verifications by appropriately-credentialed staff.
- Assess case management goals utilizing trauma-informed, person-centered, and
motivational interviewing models of care, and develop comprehensive service plans with
ongoing follow-up. Link participants to physical health, mental health, and other
supportive services from appropriate social service agencies and/or community partners.
- Adhere to housing first and harm reduction models of advocacy with participants. Utiliz
strategies from Motivational Interviewing, Seeking Safety, etc. Provide services in a
culturally competent manner and ensure trauma sensitive care.
- Educate individuals on residential lease contracts to educate them of their rights and
responsibilities. Serve as an ongoing liaison between property managers and
participants as well as between participants and neighbors.
- Collaborate with a network of CAN community providers, including, but not limited to,
outreach workers, diversion specialists, shelter case managers, housing, and other
service providers, and understand all roles and responsibilities within the CAN.
- Responsible for collection, monitoring, and reporting of required goals and outcomes.
Timely data entry into E2CT, HMIS, Smartsheet, and other reporting systems as needed
per funder requirements. Ensure that participant information is maintained with the highest level of confidentiality.
- Attend team meetings, case conferences, training workshops, and community meetings
(CAN, Community Care Team, etc.) as needed.
- Serve as a flexible team member to provide additional services or duties as needed.
Qualifications
- 2 years of case management experience required, experience with street outreach,
substance use disorder treatment, harm reduction, and/or behavioral health strongly
preferred. Individuals with lived experience are encouraged to apply.
- Strong verbal and written communication skills, including the ability to communicate
effectively across all aspects of the organization, including participants, team members,
and management.
- Computer proficiency and knowledge of Google Suite required. Experience with HMIS
and Smartsheet preferred.
- Strong organizational skills and the ability to coordinate and complete multiple tasks
simultaneously.
- Must be able to work effectively with populations that have experienced trauma,
economic poverty, incarceration, substance use, developmental disabilities, mental
health, brain injuries, literacy and numeracy issues, and/or other conditions or situations
that have impacted housing stability.
- Valid CT driver’s license and daily use of a reliable and properly registered and insured
motor vehicle. This position requires travel within the state of Connecticut. Bilingual
(English/Spanish) preferred.
- Must be able to lift up to 30 pounds and be able to physically navigate different types of
terrain.
- Flexibility to work evening and weekend shifts to conduct outreach.

Compensation & Benefits
The Homeless Outreach Case Manager position is a full-time, 35 hours per week, non-exempt
position. Hours must be flexible to meet program needs. This position is eligible for employee
benefits.
Annual pay range is $45,000-$50,000; compensation based on experience and qualifications.
