What are the responsibilities and job description for the Elementary School Therapist Specialist position at ConnextCare?
We are seeking an experienced therapist with credentials as Licensed Master Social Worker or Licensed Mental Health Counselor to join our growing Outpatient Mental Health team at ConnextCare.
As a member of our team, you will work within the APW Elementary School Based Health Center, collaborating with mental health providers, medical staff, nursing staff, and management team to ensure clinical quality of the highest standard.
The ideal candidate will have experience working with school-aged children grades K-12 and caregivers with emotional and behavioral concerns. You will be responsible for carrying a full patient load, including all necessary documentation, treatment planning, and scheduling.
Key Responsibilities:
- Counseling and therapy sessions with students and their caregivers
- Developing and implementing individualized treatment plans
- Maintaining accurate records and documentation
- Collaborating with multidisciplinary teams to provide comprehensive care
