Duties

- Assess patients' needs and develop individualized care plans in collaboration with the healthcare team.
- Monitor patient progress and communicate changes in condition as necessary to ensure effective treatment.
- Provide health coaching to patients and their families, educating them on health management and wellness strategies.
- Ensure adherence to infection control protocols.
- Maintain accurate documentation of patient care activities and outcomes.
Skills
- Experience working in a long term care environment is preferred.
- Knowledge of wound care and IV infusion practices.
- Familiarity with laboratory protocols and testing procedures.
- Ability to implement infection control measures effectively within the clinical setting.
