Job Overview
We are seeking a dedicated and detail-oriented MDS Coordinator to join our healthcare team. This role requires a strong understanding of the Minimum Data Set (MDS) and familiarity with coding systems such as ICD-10, as well as experience in managed care environments.
Responsibilities
- Conduct comprehensive assessments of patients using the MDS framework to ensure accurate documentation of patient needs and care plans.
- Collaborate with interdisciplinary teams to develop, implement, and evaluate individualized patient care plans.
- Ensure compliance with federal and state regulations regarding MDS submissions and patient care standards.
- Utilize electronic health record systems such as PCC and Simple LTC for documentation and data management.
- Maintain knowledge of coding practices, including ICD-10, to ensure accurate billing and reimbursement processes.
- Monitor patient outcomes and quality indicators to identify areas for improvement in care delivery.
- Educate staff on MDS processes, coding guidelines, and regulatory requirements to enhance overall team performance.
- Uphold HIPAA regulations to protect patient privacy and confidentiality in all aspects of care coordination.
Qualifications
- Registered Nurse (RN) or Licensed Practical Nurse (LPN) with relevant clinical experience preferred.
- Strong knowledge of MDS processes, managed care systems, and patient care practices.
- Familiarity with electronic health record systems such as PCC is a plus.
- Proficient in coding practices including ICD-9; certification in coding is a plus.
- Excellent organizational skills with attention to detail and the ability to manage multiple priorities effectively.
- Strong communication skills to collaborate with healthcare professionals, patients, and families.
