Program Coordinator (CLS-NE)
Type of Position: Full-time
Location: Queens, NY
Job Summary:
The Program Coordinator (CLS-NE) is responsible for developing, implementing, and monitoring program plans for individuals receiving services.
This position requires a strong understanding of HCBS Waiver and IRA/ICF regulations, as well as experience working with developmentally disabled individuals.
The successful candidate will be able to provide weekend coverage and work flexible hours as needed.
Key Responsibilities:
- Develop, implement, and monitor program plans for each individual receiving services.
- Supervise Direct Care personnel and Shift Supervisors in the area of programming.
- Chair and participate in team conferences for all individuals receiving services.
- Complete required documentation, including minutes, program plans, and annual updates.
- Ensure integration of services with Day Programs and complete clinical paperwork in a timely manner.
- Review monthly data sheets and ensure Residential Habilitation Billing forms are documented and submitted as regulated.
- Schedule and conduct meetings as needed, and ensure community inclusion and in-home activities are based on the interests of individuals receiving services.
Requirements:
- Bachelor's degree in a related human services field.
- QIDP status.
- Minimum two years of program experience working with developmentally disabled individuals.
- Ability to provide weekend coverage and work flexible hours as needed.
- Working knowledge of Microsoft Office Applications.
