We are seeking a highly motivated individual for the position of QIDP / Program Coordinator. This is a dynamic position that requires creativity and flexible program-solving within a collaborative team process. Have excellent communications skills, and experience working within individuals' treatment teams. The candidate will be required to pass an extensive background check and pre-employment drug screening. Knowledge of adult regulations for licensed and residential homes preferred. POSITI0N CLASSIFICATION: QIPD /Program Coordinator
WORKING TITLE: QIPD /Program Coordinator
QUALIFICATIONS: Bachelors Degree in Human service field or Education, and Two years experience working with individuals with ID in residential services.
SCHEDULE:
o Position requires that employee is willing to work the schedule as assigned of no less than 80 hours BI-weekly.
o Fluctuating Work Week (with Employment Agreement)
ESSENTIAL JOB DUTIES:
1. Supervise homemaker personal care staff and Lead Staff.
2. Oversee the operations of the service sites on caseload and assure services to the individuals are being delivered.
3. Be available to staff and consumers on an on-call basis.
4. Make decisions on call-by-call basis concerning the day-to-day
operations of the service sites assigned.
5. Serve as a chairman for the interdisciplinary team, Develop training programs for each consumer based on their needs, and assure all documentation is in compliance with all local, state and federal guidelines.
6. Monitor all programmatic documentation and assist homemaker personal care staff as needed. Complete all necessary paper work in a timely manner.
7. Assure budgets are met
8. Communication with all professional staff any concerns with consumer care to assure all services are being delivered per interdisciplinary plan which would be inclusive of assuring the consumer has all adaptive equipment that is needed for their care.
9. Oversea the living arrangements and care of the consumers residing at your designated sites. Ensure compliance with consumer's rights. Prepare assessments and reports concerning consumers.
10. Chair all admissions and discharge meetings and assure appropriate documentation of these meetings are completed in timely manner
11. Ability to complete all physical Essential Functions of Program Coordinator and HPC Duties including the ability to lift 50 pounds/ Push, Pull or drag up to 150 pounds.
