Job Title/Position: Patient Care Coordinator
Reports To: Clinical Director
JOB DESCRIPTION SUMMARY
The Patient Care Coordinator provides administrative support to the Clinical Care team by managing team schedules and the electronic medical records system. The Patient Care Coordinator may also support administrative functions of the back office to include, but not limited to billing, payroll, human resources and intake
ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES

Accurately enters a variety of data into various information systems

Executes workflow functions in electronic medical record system

Schedule patient visits in accordance to the care plan

Coordinates schedules with field-staff

Verifies accuracy of data entered through collaboration with Clinical Manager(s) and/or Clinical Director

Provides customer support to patients and colleagues via phone, email and fax

Alerts appropriate management team members regarding potential compliance issues

Maintains confidentiality of PHI and organization information at all times

Performs other specific projects relating to office procedures

POSITION QUALIFICATIONS

High School Diploma (College Degree Preferred)

At least two (2) years healthcare experience, preferably in hospice

Demonstrates autonomy, organization, assertiveness, flexibility and cooperation in performing job responsibilities

HCHB/Pointcare knowledge is preferred

Proficient in MS Office and Google Applications
