Job Functions
- The LPN/LVN assesses patient health problems and needs.
- Takes vital signs, administers medication, develops, and implements nursing care plans, and maintains medical records.
- May advise patients on health maintenance and disease prevention or provide case management.
- Understands own work area professional concepts/standards, regulations, strategies, and operating standards.
Required Qualifications
- Current state licensure as an LPN or LVN in the state of Texas
- 2 years’ experience as LPN or LVN in an inpatient or outpatient clinic setting
- Experience with Electronic Medical Records.
- Must be able to work at the Conviva Care clinic located at: 260 Tenth St. Floresville, TX 78114
Preferred Qualifications
- Hands-on professional Phlebotomy experience
- Leadership experience
- IV certification
- Bilingual in English and Spanish, with the ability to speak to patients in both languages
- Experience with HEDIS
- Knowledge of primary care medicine requirements (state specific).
Work Schedule: Full Time / 40 Hours / M-F / 8a-5p
