What are the responsibilities and job description for the Social Worker - Oncology position at Conway Medical Center?
This is a GRANT FUNDED position. Grant funded positions are subject to continuation based funding and the employee will be terminated without right of appeal if funds are discontinued or are no longer available. This position is eligible for full-time benefits.
Position Summary:
The Social Worker (SW) will provide information and assist oncology and hematology patients, their family and/or caregivers with access to internal and external resources to address practical, financial and emotional concerns across the continuum of care settings. This includes the management of the distress screening process and referral to appropriate resources to help overcome health care barriers. The SW will work with the team to get screening completed for all cancer patients at a pivotal medical visit across the continuum of care and contributes toward the creation of screening tools to address psychosocial, social, cultural and spiritual factors. The SW will evaluate the patient, family and/or caregiver coping styles and support system and to provide individual and family counseling as well as education regarding cancer support services to decrease emotional barriers. Additionally, the SW will assess and work remove barriers to care across the continuum, coordinate with appropriate resources and facilitate referrals to appropriate resources. The SW will use evidence-based practices when developing a plan that prescribes strategies and alternatives to attain the expected outcome when addressing barriers. Prepare timely documentation for medical record and maintains current activity statistics and will participate in weekly chart rounds with multidisciplinary care team to provide education, information and support regarding psychosocial issues. The SW will participate in performance improvement activities and research when possible. And work to assess, educate and (assist as needed) the patient, family and/or caregiver on the following: Social Security Disability, SSI, Low Income Subsidy, Medicaid, Medicare guidelines, COBRA, FMLA, Financial Assistance, Medication Assistance Program, Medication and Pharmaceutical Indigent Programs, Transportation Resources, Short-term Housing Resources, Advance Directives, Power of Attorney for Health Care and Wills, and Provide emotional support, provider options and facilitate transition to Hospice and in some cases, Palliative Care, depending on where the person resides.
Qualifications
Education:
- Master's degree (MSW) from an accredited school of social work required.
Experience:
- Minimum of three years of social work experience required.
- Previous social work experience in health care setting/hospital preferred.
- Previous experience in Oncology Specific Social Work preferred.
Licensure/Certification/Registration:
- License as a Social Worker (LSW) by South Carolina Department of Labor, Licensing and Regulation preferred.
