Job Summary

The CCHIP Linkage to Care Project Manager (LTCPM) will work with CCHIP staff, Stroger Hospital/Emergency Department, ACHN, and Cermak Health Services at Cook County Jail to identify & coordinate services for individuals newly diagnosed with HIV, not engaged in HIV medical care, or individuals vulnerable to HIV, for immediate linkage to primary care or PrEP services. The Linkage to Care Project Manager will develop a two-way referral system to ensure linkage to care from various portals including inpatient hospital-based and community HIV testing venues. The LTCPM will facilitate and host trainings in alignment with the Ryan White policy clarification notices, universal monitoring standards, and program standard operating procedures, inclusive of all CCHIP Ryan White HIV Aids Program (RWHAP) funded Core Medical and Supportive Services. The LTCPM will act as the CCHIP lead facilitator for upcoming pre-audit preparation, including annual review components, scopes, and RWHAP eligibility compliance. Provide direct supervision of Linkage to Care (LTC) Community Health Workers (CHW), Early Intervention Specialist (EIS), and Peer Navigators . This position is grant funded until December 31, 2025.

General Administrative Responsibilities

Collective Bargaining

  • Review applicable Collective Bargaining Agreements and consult with Labor Relations to generate management proposals
  • Participate in collective bargaining negotiations, caucus discussions and working meetings

Discipline

  • Document, recommend and effectuate discipline at all levels
  • Work closely with labor relations and/or labor counsel to effectuate and enforce applicable Collective Bargaining Agreements
  • Initiate, authorize and complete disciplinary action pursuant to CCH system rules, policies, procedures and provision of applicable collective bargaining agreements

Supervision

  • Direct and effectuate CCH management policies and practices
  • Access and proficiently navigate CCH records system to obtain and review information necessary to execute provisions of applicable collective bargaining agreements

Management

  • Contribute to the management of CCH staff and CCH' systemic development and success
  • Discuss and develop CCH system policies and procedures
  • Consistently use independent judgment to identify operational staffing issues and needs and perform the following functions as necessary; hire, transfer, suspend, layoff, recall, promote, discharge, assign, direct or discipline employees pursuant to applicable Collective Bargaining Agreements
  • Work with Labor Relations to discern past practice when necessary

Typical Duties

  • Provides direct supervision and oversight of Linkage to Care team and day to day operations.
    • Provides supervision to the staff - this includes day-to-day operations.
    • Monitors and maintains Peer Training Program in collaboration with CORE Management to meet the needs of CCHIP sites.
  • Supervises post-test HIV counseling in the Stroger Emergency Department (ED) and other CCHIP/ACHN sites as referred by providers
    • Works with CCHIP Site Team Leaders, and staff to connect patients to HIV and/or PrEP services.
    • Administers assessment in the absence of EIS staff to determine barriers to care and determine appropriate referrals, including but not limited to insurance, transportation, and other social service needs.
  • Implements the overall scope of work and coordination of new CCHIP patient appointment work to follow, schedule, and link patient to care teams.
    • Ensures staff addresses co-morbidities with care teams prior to or at the initial visit.
    • Monitors work of staff with care team to ensure implementation of patient centered care.
    • Ensures staff attends pre-clinic meetings where care coordination is discussed.
  • Works with Sexual Health Clinic Coordinator and CCHIP Program Managers to provide programmatic guidance as related to linkage and retention programs.
    • Develops linkage, referral and tracking procedures, together with CCHIP Linkage/Retention, and Data Team.
    • Collaborates with CCHIP Linkage/Retention and the Quality/Data Team to develop standardization of linkage process to include testing, outreach efforts (phone call scripts/letters/field visits), data entry, and charting.
    • Develops strategies, protocols, and processes based upon best practices, to ensure linkage and retention in care, and team community HIV awareness/testing event participation.
    • Collaborates with CCHIP retention specialists, case managers, and reentry program specialists, to coordinate retention, partner services, and PrEP services.
  • Creates an annual calendar of trainings that is inclusive of all levels of providers.
    • Receives input from providers, Peer Navigators funders, and staff to create key training focuses; revise and reconstruct trainings based on feedback and data.
    • Collaborates with CCHIP Director of Continuous Quality Improvement (CQI) to evaluate, track, and assess effectiveness of trainings.
    • Collaborates on utilizing evaluation data to create and reimagine quality improvement projects fitting for CCHIP sites and patient needs.
    • Provides monthly/quarterly reports and data to department heads and key committees.
  • Serves as the lead facilitator for annual audits and compliance visits across all level funding.
    • Familiar with RWHAP client eligibility, service standards and scopes to assure compliance.
    • Works alongside the Director of CQI, to develop and educate team on pre-audit planning and checklist of expectations and requirements.
    • Collaborates with Data Team and Director of CQI to provide guidance to the program managers and staff. Collaborates with Project Officers for all grants at the request of the Manager of Patient Centered Care, CCHIP Program Director, and CCHIP Project Management Administrator
  • Participates in CCHIP Continuous Quality Improvement Committee and Projects
  • Adheres to CCH/CCHIP Program Specific Policies & Procedures
  • Provides excellent customer service and professionalism when interacting with clients, internal, and external customers.
  • Performs all other duties as assigned


    Minimum Qualifications

    • Bachelor's degree in social work or related field from an /accredited college or university social service experience is required (Must provide official transcript at the time of interview)
    • Two (2) years of experience working with HIV/AIDS population is required
    • Proficiency using Microsoft Office Suites, Access or similar data base experience is required
    • Experience with training and facilitation of workshops both electronically and in-person is required
    • Experience with developing performance metrics such as Plan, Do, Study, Act cycles is required
    • Must be able to work nights or weekends as requested is required
    • Valid Driver's License and Insurance or other means of reliable transportation is required
    • HIV/AIDS Testing & Counseling Certification is required

    Preferred Qualifications

    • Bilingual is preferred
    • Experience working in a hospital or out-patient setting preferred. Partner Services Chicago Health Information Management System, Database (CHIMS) certification is preferred
    • Two (2) years of experience working with HIV/AIDS/or any chronically ill target populations is preferred
    • Comfortable working with high volumes of clients in a fast-paced environment is preferred
    • Experience working in primary care medical settings is preferred

    Knowledge, Skills, Abilities and Other Characteristics

    • Knowledge of HIV/AIDS and RWHAP, commitment to confidentiality, ethics, and a culturally sensitive approach to counseling
    • Excellent verbal, written communication, and interpersonal skills necessary to communicate with all levels of staff and a patient population composed of diverse cultures and age groups
    • Excellent program development, management, and leadership skills
    • Demonstrate analytical and organizational, problem-solving, decision-making, critical thinking, and conflict management/resolution skills
    • Ability to function autonomously as a worker and as a team member working with a multidisciplinary leadership team,
