Position Summary:
Manages and directs the activities of the activity department to provide a physical/psychosocially stimulation environment through an appropriate on-going program of activities directly related to the needs, wants and interest of the residents and swing bed patients of the Cook Hospital and Care Center.
The primary purpose is to plan, develop, organize, implement, evaluate, and direct the activities department in accordance with existing federal, state, and local standards, as well as our established policies to ensure that the functions are maintained in an appropriate manner.

The primary purpose is to plan, develop, organize, implement, evaluate, and direct the activities department in accordance with existing federal, state, and local standards, as well as our established policies to ensure that the functions are maintained in an appropriate manner.
The Activities Program must be directed by a qualified professional with a qualified therapeutic Specialist or an activities professional who is:
- Eligible for certification as a therapeutic recreation specialist or as an activities
- Has 2 years of experience in social or recreational program within the last 5 years,
- Is a qualified occupational therapist or occupational therapy assistant; or
- Has completed a training course approved by the state i.e., Modular Education Program
Essential Functions:
- Responsible for the overall direction and supervision of the Activities Department.
- Serves on committees as required by administration
- Maintains professional growth and development through seminars, workshops, and publications to keep current on trends and procedures affecting Activities
- Utilize computers, email, and software including, but not limited to Word, Excel, and Power Point
- Demonstrate knowledge and compliance with facility wide and departmental policies and procedures
- Actively participate in the Cook Hospital and Nursing Home Compliance Program and demonstrates compliance with required laws and regulations and ensures departmental compliance with regulatory requirements
- Actively participate in hospital mandatory education programs/in-services
- Meet attendance policy requirements
- Exercise independent judgment and discretion when carrying out the essential functions of this position
- Plan, develop, organize, implement, and direct the activity department functions of the facility.
- Develop and maintain a good working rapport with facility personnel, volunteers, residents, swing bed residents, and family members to effectively meet the needs of the facility and meet all applicable regulations.
- Oversee the development of resident/swing bed residents activity care plans, including, but not limited to, assessment, goal planning, documentation, implementation and input into resident care conference meetings and resident council. These duties being completed within specific timelines as set by regulatory agencies.
- Create and maintain an atmosphere of warmth, personal interest, and positive emphasis by working with all pertinent departments.
- Establish and maintain QAPI standards for the department.
- To hire and train qualified activities assistants and delegate, direct and supervise the activity assistants and volunteers.
- Attends and participates in Care Conferences
- Facilitates resident council meetings monthly and manages distribution of follow up and complaints of the resident per policy.
- Active member of Quality of Life, IDT, Employee Engagement, QAPI, Department Manager and other meetings as assigned.
