What You Will Earn : The anticipated starting hourly rate for this position is $30.71 -$33.21 / hour depending on experience. With a full future earning potential of $63,876.80 -$87,422.40 annually. This position is a full-time 40 hours per week and eligible for full-time county BENEFITS PACKAGE.
Job Summary :
Under general direction, this position works collaboratively with community partners including the Cook County Sherriff's Office (CCSO), North Shore Health (NSH), Grand Portage Human Services, regional crisis response providers, and in community-based settings in a pilot program to assess and develop real time co-response and intervention services to support an individual's ability to stabilize and remain in their current environment instead of requiring a higher level of care or confinement.
Essential Functions / Responsibilities :
The following duties are normal for this classification. These are not to be construed as exclusive or all-inclusive. Other duties may be required and assigned.
- Provide behavioral health crisis response education and consultation to community partners utilizing person-centered principles and interventions.
- Build strong relationships with the local law enforcement agency, mobile crisis response teams, hospital emergency department staff, treatment providers, education systems, and human services staff at Grand Portage and Cook County.
- Facilitate care coordination and communication with partners around care of individuals in crisis.
- Support PHHS and County Attorney's office with Commitment processes, as directed.
- Provide community education on behavioral health and crisis interventions.
- Provide support and education on the use of crisis planning and safety planning with partner organizations and individuals.
- Collaborate with public health and other community organizations on mental health promotion, suicide prevention and substance misuse prevention initiatives.
- Provide behavioral health crisis response to individuals and community.
Work closely with First Call for Help, Law Enforcement / Dispatch and Emergency Department services to support person(s) in mental health crisis to achieve safety and stability of the individual.
Provide crisis screening, assessment, intervention, and stabilization services under the supervision of a mental health professional.Provide short-term client intervention. Coordinate and follow up with resources within 24 hours on guidance and support provided to navigate and access services and support.Utilize existing resources such as First Call for Help, afterhours mobile crisis response, local mental health and chemical health services, regional stabilization centers, and other regional resources.Consult with Mental Health Professional / supervisor as needed when providing Mobile Crisis Response services.Attend supervision meetings and participate in team-based decisions regarding clients and services.Conduct outreach activities such as home visits, health, and wellness checks. Utilize knowledge of mental health needs to engage people with serious and persistent illness in development of skills and problem solving.Assess the risk of harm to self or others, develop safety plans, connect with resources.Connect people to resources to meet their basic needs. Provide information and assistance to individuals interested in emergency assistance, housing, financial help, and other forms of public assistance.Communicate changes in service needs and recommendations to Clinical Supervisor / Mental Health Professional, Treatment providers, Case Manager, and other members of the treatment team.Intervene as necessary for emergencies when assessing that the client is unable to manage their illness in a reasonable manner. Contact to appropriate emergency services, doctors, staff, and emergency family contacts will be made to alert them of intervention planned or taken.Ensure confidentiality of all records and interactions.Maintain appropriate therapeutic boundaries with individuals being assisted.DocumentationConduct crisis screening and / or assessment with individuals as needed.Develop crisis stabilization plan and perform all required documentation as specified in Department of Human Services (DHS) guidelines.Record all clients' contact via the Social Services Information System (SSIS).Enter case notes, case correspondence in an accurate and timely manner.Prepare service plans, completes screening documents in an accurate and timelyComplete all correspondence and communication in accordance with county, state, and federal guidelines.Utilize and consult with MN DHS and regional / local resources as appropriate.Community OutreachFoster relationships with service providers in the community, particularly with Cook County DispatchLaw Enforcement and North Shore Health Emergency Services.Have knowledge of all agency programs and their referral proceduresFacilitate external referrals to access services by making initial contact on behalf of clients and ensuring follow-up to program information and / or access to resources, as needed.Develop and maintain positive working relationships within the community.Travels to meetings, trainings and clients, homes, as needed. Must be able to transport clients in the community in some instances.Non-Essential Functions :

Education and Experience
Bachelor's degree from an accredited four-year college or university with a major in social work, psychology, sociology, or closely related field; orBachelor's degree from an accredited four-year college or university with a major in any field and one year of experience as a social worker in a public or private social services agency. Or an equivalent combination of education and skills.Preferred Qualifications
Knowledge of community resources.
Required Certifications / Licenses
Must have a valid driver's license for the purpose of client transport and travel to meetings.Knowledge, Skills and Abilities

Work is performed primarily in clients' homes, agencies, schools, community mental health agencies, treatment facilities, and court.Frequent travel required.There is potential exposure to hazardous chemicals, biohazards, and inclement weather conditions.Work may involve exposure to outdoor weather, disease, unsafe or dangerous environment in response to client and emergency situations.Direct service provision may involve interactions with persons who may be hostile, aggressive, and abusive or pose threatening conditions including physical, mental, and emotional health hazard exposure, and experiences of secondary trauma.Physical Requirements Of This Position :
