What You’ll Be Doing
The Referral Coordinator under the direction of the Practice Administrator provide direct nursing support in an outpatient urgent care and/or family practice setting.
The success candidate will be able to:
- Responsible for calling and scheduling referral appointments for patients at all 5 locations.
- Fax any requesting information and ensure proper set up paperwork has been completed.
- Follow up with the specialty offices to ensure the patient was seen and request records
- Communicate appointment dates to the patient and ensure they have a follow up appointment with the referring provider
- Communicates with primary care provider (PCP) to inform of any delays or barriers to patients care.
- Communicates with Supervisor when unable to process all referrals or follow-ups to avoid delays.
- Carries out patient discharge processes; clarifies and provides patients with follow up care instructions and referrals.
- Ensure all results and reports from either in-house testing or outside testing is in the chart prior to the physician seeing the patient or according to the physicians request.
- Will be part of the Patient Service Team and utilized as an overflow operator.
- Perform any and all other duties as assigned

- At least1 year of experience in a medical practice or urgent care setting preferred
- Knowledge of examination, diagnostic and treatment procedures
- Exceptional organizational and multi-tasking abilities
- Effective communication and interpersonal skills
- Proficiency in using electronic health record (EHR) systems and other relevant software
- Problem-solving aptitude and a proactive approach to addressing challenges
- Attention to detail and a commitment to delivering high-quality patient care
- Skill in establishing and maintaining effective working relationships with physicians, patients, employees and the public
