Short Description
The Clinical Pharmacy Specialist provides advanced clinical pharmacy services in various specialties to ensure optimal safe and cost effective therapeutic regimens in collaboration with respective specialty disciplines.
Provide clinical pharmacy services for optimal cost effective patient care. Reviews physician orders to determine therapeutic rationale and safe patient regimens.
Responsible for oversight and monitoring/data collection of patients
Educate health professionals, pharmacy staff, patients and students through in-services and written/verbal materials. Provides preceptorship and mentorship to students, faculty and residents
Multidisciplinary collaboration for policies, procedures, guidelines and protocols
Collaborates with specialty areas to ensure appropriate drug utilization for cost effective therapy
Assists with cost containment initiatives through formulary review and drug use evaluations
Provides input and supports pharmacy department initiatives in the daily collaboration with the clinical teams
Effectively communicates recommendations and education to pharmacy and the healthcare team
Experience Required
3-5 years preferred.
Education Requirements
Pharmacy Doctorate Degree.
License/Certification Requirements
New Jersey Board of Pharmacy Licensure.
Special Requirements
Hospital pharmacy experience (or clinically relevant experience)
PGY2 required in specialty or equivalent clinically relevant experience
PGY1 required
