Short Description
The Cancer Navigator is a specially trained professional who guides patients through the diagnosis, treatment, and follow-up process of cancer. This position will focus on all cancer types. The Navigator functions as a resource to a multidisciplinary team of specialists as an advocate, interpreter, educator and counselor for the oncology patient, their family and referring public. The Navigator is responsible for ensuring patients with an oncology diagnosis receive quality and comprehensive coordinated services in a timely manner. The Navigator will coordinate care throughout the continuum in collaboration with the multidisciplinary team. The scope of this work includes facilitating dynamic scheduling for patients, triage of services and patients as needed, coordination of care, integration of clinical activities across various specialties in cancer care and follow up from diagnosis to survivorship.
Duties/Responsibilities
Experience Required
- Oncology experience required
- Working knowledge of federal, state, and local laws as well as Joint Commission requirements that govern health care and Oncology care
- Knowledge of community resources
- Skilled at navigating EMR for appropriate knowledge and documentation experience required.
Education Requirements
- Bachelor of Science in Nursing (BSN) required
License/Certification Requirements
- Current NJ-RN License
- Current Oncology Nursing Certification required (OCN or ONN-CG)
- BLS required
- Oncology Navigation Certification preferred
- The Nurse Navigator will become certified in Navigation within 2 years of hire, if certification is not already attained.
