Job Description
The Case Manager Registered Nurse will play a key role in coordinating patient care across the continuum of illness, from hospital admission to post-discharge follow-up. This individual will work closely with the interdisciplinary Patient Care Team and Primary Care Physician to develop and implement a comprehensive plan of care that addresses the patient's physical, emotional, and social needs. Key responsibilities include conducting admission and continued stay review, monitoring utilization management and monitoring process, identifying trends and patterns of resource use and patient care, and ensuring seamless transitions of care.
Requirements
To be successful in this role, candidates must have a Baccalaureate degree in Nursing from an accredited school or university, or equivalent RN degree with commensurate clinical experience. A minimum of 2-3 years of experience in a hospital setting is required, as well as a minimum of one year of experience in Case Management/Utilization Management or Home Care. Full-time employment with weekend and holiday commitments is necessary.
