The behavioral health professional in this role will provide Supported Employment and Housing Services to members with a diagnosed Serious Mental Illness (SMI) who are seeking to obtain and/or maintain competitive employment in the community. Responsibilities include:
- Following the 7 Supported Employment Core Principles of Substance Abuse and Mental Health Service Administration (SAMHSA) zero exclusion, member preference, rapid job search, competitive employment, integrated services, long-term supports, and benefits planning.
- Adhering to the philosophy of normalization and community integration as appropriate.
- Attending meetings on assigned caseload and communicating with the clinical team regarding concerns and accomplishments in a professional manner.
- Being aware of individuals’ physical restrictions, medical concerns, and normal safety routines.
- Ensuring all utilized equipment is properly maintained.
- Functioning as an integral part of an interdisciplinary team to develop individual housing plans.
- Conducting intakes and assessments, determining eligibility, and developing individualized case plans.
- Collaborating with housing specialists to find permanent housing options for program participants.
- Conducting job development and job search activities directed toward positions that align with the interests and uniqueness of the members on their caseload, following Copa Health’s Supported Employment Program principles.
- Linking individuals and families with community-based housing using the Housing First/Permanent Supportive Housing evidence-based practice.
- Educating and maintaining knowledge of community resources that serve individuals with disabilities.
- Managing financial assistance funds (security deposits, rent, etc.).
- Determining and recommending financial assistance levels for eligible participants to the supervisor.
Candidates must meet the following criteria:
- Education & Experience:
- High School Diploma/GED plus four (4) years of full-time direct care experience in behavioral health OR
- Bachelor’s degree (highly preferred).
- Licensure & Certifications:
- Must obtain a Fingerprint Clearance Card issued by the Arizona Department of Public Safety within eight (8) weeks of hire.
- Transportation Requirements:
- Must have reliable transportation, a valid AZ Driver’s License, and state minimum insurance coverage.
