- Full-Time, 40 hours per week

- Salary: $43,700-$47,800 annually

- Location: EmotiZone Gilbert OP Office
- Bachelor’s degree in social work, Counseling, Psychology, or a related field OR equivalent field relatable experience
- Minimum of 2 years of experience in direct care adolescent behavioral health services with community-based outreach and case management experience
- Excellence in customer service
- Knowledge of HIPAA regulations and PHI security requirements
- Proficiency in Microsoft Office Suite, including Word, Excel, and Outlook.
- Ability to thrive in a fast-paced, dynamic environment while managing multiple priorities.
- Excellent interpersonal, verbal, and written communication skills
- Strong ability to foster positive and collaborative relationships with diverse stakeholders,

- Bilingual preferred
- CPR/First Aid certification (or willingness to obtain upon hire)
- Must provide proof of freedom from infectious tuberculosis before starting employment
- Ability to obtain and maintain a Fingerprint Clearance Card as state regulations require
- Proficiency in crisis intervention and ability to assess safety concerns during intake
- Knowledge of Arizona state regulations for Behavioral Health Medical Facilities
- Competency with behavioral health assessments, treatment planning, and clinical documentation
- Strong organizational skills
- Ability to work with adolescents experiencing behavioral and mental health challenges
- Excellent communication skills for engaging with families, referral sources, and internal teams
- Proficiency in electronic health records (EHR) systems and documentation requirements
- Ability to handle crisis situations calmly and professionally
- Contributes to a positive thriving work environment
- Provide direct care/services to clients and their families in home, school, office or community
- Conduct comprehensive behavioral health assessments, develop and deliver services based on tailored treatment plans
- Provides Life Skills training, Family Support services, Counseling, and Case Management
- Manages crisis effectively
- Provides services in a group format as needed
- Works effectively with supervisors, in a team format, and independently
- Responsible for maintaining and managing a case load of 50
- Provides supportive services and case management follow up for Respite clients as needed
- Implement evidence-based practices to foster client progress effectively
- Collaborate with families, schools, and community partners to facilitate coordinated care and grow the program reach in the community
- Ensure accurate and timely clinical documentation in compliance with regulatory and agency standards
- Required to meet productivity standards set
- Require producing clinical records proficiency at 85% or above
- Assist in developing and implementing programs focused on quality improvement
- Ensure timely submission of reports, audits, and other compliance documentation
- Complete all required training processes
- Represent the organization in community settings and build partnerships with external stakeholders
- Setting: Outpatient clinical setting, Community Based, Client Home or School
- Physical Demands: Prolonged sitting, standing, walking, and occasional lifting of up to 25 lbs. May experience inclement weather; heat or cold. Hazards working with unstable population
- Schedule: May require occasional evening or weekend availability based on client needs. Required to be on call and carry a cell phone
- Travel/Training: Must have reliable transportation, valid AZ drivers license, pass MVD screen. Driving a company or personal vehicle in the community is required
