The ACT Independent Living Specialist (ILS) is part of the Assertive Community Treatment (ACT) Team, providing direct treatment, rehabilitation, and support services to consumers in a community setting. The ACT Independent Living Specialist (ILS) is part of the Assertive Community Treatment (ACT) Team, providing direct treatment, rehabilitation, and support services to consumers in a community setting. Responsibilities include assessing consumer needs, developing individual service plans, advocating for consumers, liaising with families and agencies, and providing crisis intervention, including rotating on-call, weekend, and holiday coverage.
MINIMUM QUALIFICATIONS:
- High School diploma/GED and four plus (4 ) years direct care experience, or;
- Any combination of direct Behavioral Health experience/education totaling five (5) years of experience is acceptable. Example: Bachelor’s degree with one (1) year of experience.
- One (1) year education and one (1) year professional experience in, substance abuse treatment and recovery.
- Requires on call rotation, this may include weekend / holiday work.
- Requires the use of your personal vehicle.
- Successfully completes a pre-employment drug screening.
- Ability to obtain and maintain a Level One Fingerprint Clearance Card and any other required background checks.
- Works as part of a clinical team to support recovery and inspire hope for members while assisting them in achieving their service plan goals.
- Provides continuous assessment and feedback regarding their progress in relation to their service plan, regardless of the member’s current location.
- Meets with members in the community following established policies, procedures, and clinical protocols to offer supportive counseling, skill development, and other necessary support services.
- Collaborates with the member’s Prescribing Clinician, Nurse, and other clinical team members to ensure the timely development and implementation of the individual’s service plan, including all required assessments and crisis plans.
- Coordinates with all parties involved in the member’s treatment as per policies and clinical protocols, including but not limited to attending inter- and intra-agency meetings, communicating via phone or written correspondence, and ensuring effective service and support delivery as outlined in the service plan.
- Responsible for transporting members to necessary services as indicated in their service plan.
- Serves as the primary contact for a designated percentage of team members, ensuring accurate, thorough, and up-to-date documentation in medical records.
- Additional job duties may be assigned as needed to ensure compliance and quality services within the program or department, aligning with Copa Health productivity expectations.
