We are looking for a CCBHC Outreach Worker to join our team! Under the direct supervision of the Program Coordinator the CCBHC Outreach Worker will work as a member of both the CCBHC team and the patients treatment team to promote positive behavioral and physical change for patients. CCBHC Outreach worker is responsible for clinical oversight of patient care, and to the team as it relates to the delivery of services, including the assessment and service planning process, and monitoring and tracking CCBHC related services. Provides and coordinates all covered services, (e.g. case management, outreach, crisis intervention, counseling, in addition to the CCBHC nine core services) to address patient needs. The Outreach Worker will be responsible for identifying, locating and engaging people in treatment and integrated health services. Must be able to follow up with patients who have disengaged from services or fail to show for their intake.
Shift:
- Sunday 7am - 11am, Monday, Tuesday and Wednesday 7am - 8pm
Minimum Qualifications:
- Bachelors or master’s degree, no experience necessary or
- Bachelor’s degree in a non-behavioral health related field plus one (1) year experience providing behavioral health services or
- Associate degree plus two (2) years behavioral health work experience or
- High School Diploma or GED plus four (4) years of behavioral health work experience.
Additional Requirements:
- Must be 21 years of age (automobile insurance requirement) and possess a valid AZ driver’s license with a clean or acceptable driving record.
- At time of job offer, submission of 36 month (three year) driving record from DMV will be required. These standards must be maintained throughout employment and up-to-date driving records are reviewed regularly for current employees.
- Applicants must be able to pass a pre-employment drug screen and background check.
- Level 1 Fingerprint Clearance Card
