General Purpose:
MDS Coordinators oversee and facilitate the completion of resident assessments at nursing facilities. Throughout the assessment and while planning care, they ensure compliance with Federal and State regulations. An MDS Coordinator is usually a member of the upper management team as they’re responsible for managing the assessments. They work in conjunction with the management team and is part of the on call rotation.
Qualifications:
· Must be a licensed RN or LPN/Current and valid state nursing license
· Must have clinical experience in a healthcare setting
· Demonstrates basic computer skills
· Shows proficiency in MDS 3.0
· Demonstrates knowledge of state and federal regulations
· Detail-oriented

· Medical terminology knowledge
Duties and Responsibilities:
· Administer Patient Assessments (Sets The assessment schedules and assure assessments are completed in an accurate and timely manner.)
· Develops Care Plans
· Participates in the admission process in terms of their nursing needs and appropriate placement.
· Determines RUGS and expense associated with a potential admission
· Reviews all applications for admission, followed by on-site or phone assessment and communicates the results of assessment to the Admissions Coordinator
· Completes and assures the accuracy of the MDS process for all residents
· Maintains current working knowledge of Medicare criteria, serving as a resource for nursing staff and communicate changes in regulations
· Monitor Case Mix Index scores, looking for potential risks and/or changes that may affect Medicaid reimbursement
· Facilitate problem solving for complicated admissions
· Monitor Medicare assessment schedules and nursing documentation to ensure accuracy and timely submission
· Creates a schedule for all Medicare and Medicaid
