Integrity Healthcare Communities is looking for an MDS Coordiantor for one of our skilled nursing facility located in Anna IL. We offer great benefits and a great team work environment. Must be dependable, highly motivated and personable to work with residents and families.
MDS Coordinators Will-
- Direct the Residents Assessment Process through completion of the Minimum Data set (MDS) and Care Area Assessments (CAA's)
- Lead the development of individualized resident Care Plans
- Write restorative programs and monitor outcomes
- Identify the needs of the resident and projected outcomes; as required by Federal and State Regulations.
Requirements
- Must possess a valid IL. RN or LPN License.
- The ideal candidate will have at least 2 years previous experience with both PPS and OBRA assessments.
- Strong understanding of the Medicare and Medicaid Reimbursement System is preferred.
- General knowledge of rehabilitative and restorative nursing, laws and regulations governing Long-term Care, computer proficiency, ability to manage deadlines, and strong analytic skills.
Pay is very competitive and commensurate upon experience and education.
