What are the responsibilities and job description for the MDS Coordinator position at The Heritage Center at Holiday Health Care?
The Heritage Center of Holiday Health Care is a privately owned and operated nursing facility located on the North side of Evansville. We are currently seeking an experienced and detail-oriented Part time MDS Coordinator to join our team! This position will complete all MDS assessments for residents on the nursing unit and participate in/direct the resident care conference.
Qualifications:
- Graduate from an accredited school of nursing.
- Current nursing license in the State of Indiana.
- Strong interpersonal skills and ability to effectively communicate with other disciplines, residents and families.
- Experience in nursing of the long term care resident.
- Working knowledge of the RAI manual, case mix index, ICD-10 codes, and strong assessment skills with attention to detail.
- Experience with computers and MDS submission.

Responsibilities:
- Receives report daily.
- Listens for actual and anticipated changes in resident care.
- Completes unit rounds to assess resident function and staff interaction including mealtimes.
- Obtains additional information from direct care team members.
- Coordinates and oversees the care plan process by ensuring the MDS is timely based on State and Federal guidelines and ensuring the care plan reflects the resident’s status via revision of care plan.
- Monitors completion of ADL monitor corresponding with the MDS reference date
- Completes nursing sections of the MDS.
- Signs completed MDS indicating sections completed.
- Collaborates with all disciplines including physician to ensure an interdisciplinary plan of care is developed for each resident.
- Ensures the care plan is linked to the MDS via the RAP guidelines and summary. Participation in care conference for each scheduled resident.
- Provides education (pertinent to the care provided) to resident, family, and unit team members.
- Maintains information in a confidential manner.
- Is accountable when made aware of suspicions and does not respond to.
- Maintains positive working relationship between all team members.
- Inputs ICD-10 Codes.
- Reviews care plans, Braden’s Scale, Pain and Falls Risk assessment, etc. quarterly.
- Transmit MDS in a timely manner.
- Maintains own competency through participation in facility in-service training, continued education and media sources.
- Promotes positive guest relations
- Promotes positive image of long-term care and demonstrate the facility's philosophy and objectives.
- Coordinates with Restorative Nurse, restorative/Maintenance Programs.
- Participates in departmental meetings.
- Participates in staffing on-call for assigned units.
- Performs other duties deemed necessary by the Administrator or DON.
- Employees must meet the attendance and scheduling requirements of their position as an expected and essential duty of the position.
- Due to the nature of our work, all employees are expected to demonstrate a reasonable degree of flexibility in covering extra shifts/hours, working over-time, and holidays.
