This position is the heart of the clinical team.. The Case Manager is responsible for ongoing communication with referral sources and other team members using a high degree of productivity and confidentiality. This position represents CORE to potential and current referents. This position has extensive client contact and requires empathy, compassion and understanding.


This position is Monday - Friday; 2p - 10p.

  1. Provide and assist new residents with clinical admission paperwork.
  2. Complete the Referent Contact Form and send to referent via fax or email.
  3. Obtain Authorization for Disclosure for all potential individuals involved with the client.
  4. Complete weekly contacts with referents per signed Authorization for Disclosure.
  5. Assist residents with housing search as needed, including contact with Sober Living facilities and associated applications.
  6. Arrange continuing care appointments.
  7. Document all work in EMR.
  8. Rotate on call with clinical team.
  9. Verify resident medication supply for post-discharge.
  10. Send discharge summary to appropriate referents and send to Clinical Manager for review and internal routing.
  11. Audit completed charts.
  12. Provide education to residents regarding satisfaction surveys, QR code.
  13. Complete 30/90/180 follow up contacts with previous CORE Treatment Services clients.
  14. Work collaboratively with Clinical Residential Manager to accept Residential referrals and admits, maintaining Resident Roster.
  15. Complete pre-screens for Residential admit.
  16. Provide backup to Residential clinical staff.
  17. Lead client lecture/group daily.
  18. Attend staff meetings as required.
  19. Perform other duties as requested by Leadership.

Qualifications & Skills Required:

1.Must have computer skills, including experience with PC computers, Microsoft Office & Word, Excel, PowerPoint, and image files.

2.Ability to manage, schedule, and administrate multiple tasks and assignments.

3.Excellent verbal and written communications skills.

4.Must be a teachable and contributing team member.

5.Ability to work in a fast paced and potentially stressful environment.

6.Ability to be a problem-solver and self-starter.

7.Follow agency protocols and policies.

8.Must be flexible to change, have ability to work on a variety of assignments and/or projects simultaneously.

9.Must be able to work for a variety of personalities and requests, sometimes on short notice.

10.Must have excellent verbal, written communication skills and interpersonal skills.

11.Must be able to handle a fast paced work environment.

12.Must protect the reputation and integrity of others through strict confidentiality.

13.Demonstrate positive and energetic influence throughout the organization and exhibit honesty and trustworthiness in all aspects of the job.

14.Must exhibit positive, friendly and sincere interactions to guests, vendors, volunteers and staff.


Education:

Associate’s or Bachelor’s degree in AODA Counseling, Psychology, Education, Social Work or related field, or in process of obtaining.

Application for or completed SAC-IT, SAC or CSAC, preferred but not required.


Experience:

Experience in recovery or client care field preferred but not required.

1 – 5 years experience in case management.
